Information collected: company/organization, contact name, email, phone, optional information, document name, and request and delivery times
Purpose: document delivery, download-history management and responding to related requests
Retention: 90 days from the request date. Expired records are deleted when a new request is submitted or an administrator logs in or queries records. Histories are emailed to designated staff; email copies are managed and deleted separately.
